Fingerprint Crimial History Records Check Application Form This form is needed for employees that request access to restricted areas that requires a fingerprint/background check be performed.  This form must be accompanied by a company check or money order in the amount of $45 payable to the New Orleans Aviation Board (NOAB) Privacy Act Notice This form is required for all employees. Escort Authority Access Form Employees wishing to add the "E" or Escort Authority to their badge must complete this form and obtain the company's sponsor's approval. Key Maintenance Form This form will need to be submitted whenever an employee requests a new key from the Security Office. Lost, Stolen or Expired ID & Key Form When an employee loses a key or their ID badge this form must be completed and submitted to the Security Office before they will be reissued.  Fees may be associated as well as re-training.
