Both flights will be in the same terminal. if everything is on the same reservation you just have to walk from your arrival gate to your departure gate.

To get wheelchair service you call United and ask for Wheelchair service. That is just enough time if the flight from Phoenix is on-time, but you likely will have to go from Terminal C to Terminal B.
